Sensory Profile Assessment
A sensory assessment can support you to explore and understand which sensory input in your environment is supportive to your emotional regulation and which sensory input is overwhelming.
The report provides recommendations and accomodations that support you/your child in their/your environments & in sensory processing.

Functional Capacity Assessment
This assessment and report inform the National Disability Insurance Scheme (NDIS) of what you/your child's current needs are from a physical, cognitive, and psychosocial framework to inform funding allocation on plans.
The process is as follows:
Fill out a questionnaire
Attend a session with the OT for further clarification about current needs.
Complete standardised assessments.
Provide relevant reports or documentation from other health professionals or learning institution about yours/your child's development.
The OT collates this information into a report and then provides you with a feedback session on the report.
Diversum Health Group does NOT complete FCA for home modifications, specialist disability accomodation (SDA).

NDIS Review Report
NDIS progress reports are sometimes requested by your Local Area Co-ordinator to inform the NDIS of you/your child's development in working towards your/your child's goals at the end of a plan.
These reports are required to be handed in 8-weeks before the plan is due to finish, therefore it is recommended that this service is scheduled at least 12-weeks prior to the end of plan.
